We are pleased to see you as a patient. Here are the registration forms you will need. You may print them out and bring them with you to your appointment.
We will take a detailed medical history at your first visit. Remember to bring a list of all of your current medications, both prescription and over-the counter. If you have several things you want to discuss with Dr. Spencer, feel free to write them down and bring the list!
